ADHD Right to Choose Referral Process

Steps to Get Referred

  1. Choose a provider
    Download and select a Right to Choose provider from the table.
  2. Complete questionnaire
    Download and fill in the ADHD Self-Report Form |  Adult: ASRS or Child: SNAP-IV
  3. Book a GP appointment 
    Take: 
    Your completed form
    The name of your chosen provider
  4. GP referral
    If appropriate, your GP will complete and submit the referral.
  5. Wait for provider contact
    The provider will contact you to arrange an appointment. 

Important Information

  • NHS funding
    Right to Choose is NHS-funded. This may change - if your provider stops NHS work, you may need to switch to another provider.
  • Waiting times
    Timeframes vary and can be long. Check the provider’s website for the most up-to-date estimates.
  • Shared care requirements
    You usually need to stay with the provider for at least 3 months before a shared care agreement is considered.
  • Prescribing costs
    If your provider does not offer NHS prescribing, you may need to pay more than the standard prescription charge.
  • Ongoing monitoring
    You must attend:
    • Annual specialist reviews
    • Physical health checks (blood pressure, heart rate, BMI)

Failure to comply may result in medication being stopped.
